The accident occurred around 14:30 on Wednesday 13 June 2012. The Department of Mineral Resources ("DMR") inspectorate in the North West Province has been alerted and an initial investigation has been conducted. The inspectorate has issued a Section 54 instruction, stopping all production at Tau Lekoa. The management of Tau Lekoa is working closely with the DMR to minimize the duration of the stoppage on production at Tau Lekoa, while a full investigation continues.
